Birth and Death Certificates are public documents available according to a specific fee schedule. Certified birth and death certificates can be purchased for $23.00 per copy, and records date from December 1908 to the present day. Certificates may be obtained through the Warren Health Department or the Ohio Department of Health's Bureau of Vital Statistics (614.466.2531).

 

When a copy of Birth or Death Certificate is required, you may apply in person at the Main Avenue office or forward your request via the U.S. Postal Service or private carrier. When sending a request through the mail, please use the following instructions:

• Print or type legibly

• Enclose a self-addressed, stamped envelope for faster service

• Provide the exact and full name, date of birth and/or death, and any supporting family information, such as mother or father's name, funeral director, location within county where birth occurred (City, Village, Township)

• If immediate delivery is needed, enclose a post-paid overnight return envelope from the service used to deliver your request

• Supply a letter with the specific number of copies requested

• Be sure to enclose payment for the number and type of certificates requested. PERSONAL CHECKS ARE NOT ACCEPTED BY THIS OFFICE.

• Enclose your full mailing address and telephone number in case the office must contact you for further information

• Forward your request to the above-listed address
