DEATH & BIRTH CERTIFICATES

Death and Birth Certificates are considered public documents and are available to the public according to a specific fee schedule. They are also available by contacting the Ohio Department of Health, Bureau Of Vital Statistics at (614) 466.2531. Records date back to December 1908 to the present.

Copies may be purchased as either "Certified" which will depend upon your need, or use of the certificate. Certified copies have a raised seal and are required as positive proof of death or birth.

When a copy of a Death or Birth Certificate is needed you may visit our offices, or forward your request via the U.S. Postal Service, United Parcel Service, etc.. When requesting through the mail please follow the following instructions:

{ Print or type legibly and include;
{ A self addressed, stamped envelope for faster service.
{ The exact and full name, date of birth and/or death, and supporting family information such as mother or fathers name, funeral director, location within Trumbull County where birth occurred (City, Village, Township) , etc.
{ If immediate delivery is needed send a post-paid overnight return envelope from the service used to deliver your request.
{ A note or letter requesting a specific number of certified or non-certified copies.
{ Payment for the number and type of certificates requested. NO PERSONAL CHECKS OR CASH WILL BE ACCEPTED THROUGH THE MAIL.
{ Your full mailing address and telephone number in case we may need to contact you.

{ Forward your request to the address listed on the form.

Costs: Certified copies, per copy………..$23.00 each.

CITY OF WARREN FEE SCHEDULE

The following permits and/or licenses are sold through the Health District. Please select the Chapter hyper-link for detailed and specific information from the Warren City Codified Ordinances. This information will be helpful in determining the need for specific licenses and/or permits, explain costs, etc.

{ Chapter 723 Garage Sales. Three day maximum, one per year, $3.00.
{ Chapter 725 Going Out Of Business Sales. Jewelry stores only.
{ Chapter 726 Going Out Of Business. All other sales.
{ Chapter 729 Mechanical Amusement Device. Pool tables, games, etc.
{ Chapter 735 Peddlers. Door to door solicitation, $100.00 per year, plus $10.00 per sales representative.
{ Chapter 735.14 Flea Markets. $150.00 per year.
{ Chapter 745 Taxicabs. Drivers $20.00, Company $100.00 plus vehicle stipulations. Franchise licenses available.
Chapter 752 Massage Establishments. Ohio Licensed Medical personnel are exempt.
